They are essentially the same, but basal metabolic rate is a measure of energetic expenditure by only necessary bodily functions while resting metabolic rate is basal metabolic rate + any other processes that occur while resting.
For people who exercise regularly, not at all. For a few, it can be more difficult in the last 3 minutes, but you should not be worked to your limit unless you are doing a VO2 max test.
